2026-2027 Transportation Opt Out Form
OSTA is asking parents/caregivers who wish to opt out of transportation for the 2026-2027 school year to complete the following form.
Please read carefully

Please complete this form if you do not intend to have your child use the school bus starting in September 2026.

If your circumstances change, leaving you needing transportation at anytime, notify OSTA by completing OSTA's Transportation Issues Form. Please be advised it may take up to three business days to re-establish transportation services. Note that OSTA will review the request to ensure the student is eligible for transportation and meets all terms and conditions before assigning transportation.

Please note that you are opting out of transportation services for the 2026-2027 school year only. OSTA will reinstate your transportation services for the following school year unless indicated by the parent/caregiver and/or a re-submission of this opt-out form occurs.

Joint Custody and Transportation to Two Homes: Parents/caregivers of students applying for joint custody (variable) transportation are encouraged to review OSTA's variable transportation policy, procedure and application form. Separate forms must be completed.
